Rate of Patients Who Experience a Wrong Site, Side, Patient, Procedure, or Implant

An error in surgical site, side, patient, procedure, or implant used during a surgery is rare, and an indication of poor quality care. Reasons for these kinds of errors during surgery include failure to accurately mark a patient during surgical preparation, mishandling of documentation, and poor communication with the patient. This measure assesses whether the surgery center has any patients experience any one of these errors during the reporting period.

  • Outpatients (Patients who are admitted and discharged on the same day)
  • Adults
