Medication Reconciliation

Medication reconciliation is the process of identifying and maintaining an accurate list of all medications a patient is taking. Accurate medication reconciliation can help prevent medication errors that could seriously harm a patient. This measure assesses whether the hospital adheres to an endorsed medication reconciliation protocol, which includes having a pharmacist review the list of medications a patient was taking at home and ensuring the list matches the patient's admission and discharge orders, and the hospital's rate of unintentional medication discrepancies.
