Medication Reconciliation
Medication reconciliation is the process of identifying and maintaining an accurate list of all medications a patient is taking. Accurate medication reconciliation can help prevent medication errors that could seriously harm a patient. Hospitals are asked to report on their adherence to a medication reconciliation protocol endorsed by the National Quality Forum (NQF), which includes having a pharmacist review the list of medications a patient was taking at home and ensure the list matches the patient's admission and discharge orders.
